Want to Save Time?

To save time on your first visit we have made it easy to “do the paperwork”.

Just simply PRINT OUT “Confidential Patient History” and the "Informed Consent" form, fill it out and bring it in at your first appointment. It is the exact same form you would have been given by our receptionist. We will then insert your completed history into a file for you here at the clinic.

If you choose to fill out the form at our office, you can do that too.

Wascana Remedial Massage Centre assumes no responsibility for confidentiality of the Patient History form until it is submitted to the clinic on the first visit of the client / patient.
